How to Remove "the Disk is Write Protected" error for USB pendrive

QUESTION?

 

I can't format my USB pen Drive, it shows the error as "The Disk is Write Protected"
I tried various methods to solve this but all are not working.

There's no switch in the USB disk.
I am using Windows XP SP3,Vista, Win7 and Win8.

Please give me solution as earliest as possible.
